Its surface connects with the femur\u2019s trochlea, which is hollow so the kneecap stays centered during knee movements. It is drawn outwards by traction from the quadriceps and the external retinaculum, and by the discrepancy between the ATT axis and the trochlea\u2019s neck axis (TA-GT). If medical treatment is unsuccessful, surgery may be considered.<strong><br \/>\n<\/strong><\/p>\n<p>[\/vc_column_text][vc_column_text]<\/p>\n<h3 style=\"text-align: justify;\"><strong>Bone corrections<\/strong><\/h3>\n<p style=\"text-align: justify;\"><strong><img decoding=\"async\" class=\"size-full wp-image-7279 alignright\" src=\"https:\/\/chirurgie-orthopedique.info\/wp-content\/uploads\/2021\/04\/chirurgie-Rotule-correction-osseuse.png\" alt=\"Chirurgie de la rotule : Corrections Osseuses\" width=\"230\" height=\"168\" srcset=\"https:\/\/chirurgie-orthopedique.info\/wp-content\/uploads\/2021\/04\/chirurgie-Rotule-correction-osseuse.png 230w, https:\/\/chirurgie-orthopedique.info\/wp-content\/uploads\/2021\/04\/chirurgie-Rotule-correction-osseuse-125x91.png 125w\" sizes=\"(max-width: 230px) 100vw, 230px\" \/><\/strong><\/p>\n<p style=\"text-align: justify;\"><strong>Transposition of the anterior tibial tuberosity is a procedure that realigns and pulls down the off-center kneecap. <\/strong>The tibial tuberosity is removed and screwed inwards and\/or lower. Trochlear recession is performed in order to improve patellar stability by hollowing out the flat trochlea, making it concave, and facilitating proper kneecap retention. These procedures are not common, and should be reserved for cases of severe trochlea dysplasia (convex trochlea).<\/p>\n<p style=\"text-align: justify;\"><strong>\u00a0<\/strong><\/p>\n<p>[\/vc_column_text][vc_column_text]<\/p>\n<h3 style=\"text-align: justify;\"><b><strong>Patellofemoral ligaments surgery<\/strong><\/b><\/h3>\n<p style=\"text-align: justify;\"><strong><img decoding=\"async\" class=\"alignright wp-image-7282 size-full\" src=\"https:\/\/chirurgie-orthopedique.info\/wp-content\/uploads\/2021\/04\/chirurgie-rotule-plastie-ailerons.png\" alt=\"Chirurgie de la rotule :\u00a0Plastie des ailerons\" width=\"180\" height=\"144\" srcset=\"https:\/\/chirurgie-orthopedique.info\/wp-content\/uploads\/2021\/04\/chirurgie-rotule-plastie-ailerons.png 180w, https:\/\/chirurgie-orthopedique.info\/wp-content\/uploads\/2021\/04\/chirurgie-rotule-plastie-ailerons-125x100.png 125w\" sizes=\"(max-width: 180px) 100vw, 180px\" \/><\/strong><strong>Patellofemoral ligaments surgery can be performed alone\u00a0if there are few bone anomalies, or it can be combined with ATT transposition.<br \/>\nIf the kneecap is painful and only barely unstable, we cut the external retinaculum and tighten the internal retinaculum with subcutaneous stitches through arthroscopy.<\/strong><\/p>\n<p style=\"text-align: justify;\">A new procedure was developed a few years ago in order to treat severe patellar instabilities. It is a reconstruction of the medial patellofemoral ligament that uses the gracilis tendon (MPFL: Plasty). The gracilis tendon is taken with a stripper (as for ACL surgeries). It is secured with two absorbable anchors to the kneecap\u2019s inner edge, and then both ends are tightened in a tunnel by an absorbable screw on the inner edge of the inner condyle. There are only three incisions, 1cm long.
